Immunization and Physical Exam Requirements

Does your child need vaccines?

  • Call your child's doctor to schedule an appointment as soon as possible
  • Call your child's School Based Health Center
  • Call the Parent Information Center to make an appointment with the nurse

Do you have a child entering Pre-K or Kindergarten? Have they received a lead screening test?

Massachusetts requires all children have lead screening test before they can enter daycare, Kindergarten, or preschool.

Do you have a child entering 7th or 11th grade this year? If so, are you aware of the new meningococcal conjugate vaccine requirement?

Meningococcal Requirements

Grade 7

1 dose; 1 dose MenACWY (formerly MCV4) required. Meningococcal B vaccine is not required and does not meet this requirement.

Grade 11

2 doses; second dose MenACWY(formerly MCV4) must be given on or after the 16th birthday and ≥ 8 weeks after the previous dose. 1 dose is acceptable if it was given on or after the 16th birthday. Meningococcal B vaccine is not required and does not meet this requirement.

Does your child want to participate on a WPS athletic team? Have you provided the school nurse with an updated physical?

The purpose of a sports physical is to ensure the safety of the student and provide guidelines for those with health needs. The physical exam needs to be within 13 months of starting any sport.
